PATHS [Permanent Connections, Academics, Training and Employment, Housing Social and Emotional Well-being (PATHS) Program]

In December 2020, CW Solutions was selected as only one of two providers to operate the PATHS program in Wisconsin as part of a new five-year grant. Our program covers the NorthCentral Region - covering the following nine counties: Adams, Forest, Langlade, Lincoln, Marathon, Oneida, Portgage, Vilas and Wood. We anticipate the program will begin serving youth by April 2021.

PATHS is a Housing First program for youth age 17 - 22 who have aged out of court-ordered care without being reunified with their family. PATHS  has five core service elements:

  1. Permanent Connections
  2. Academics
  3. Training and Employment
  4. Housing
  5. Social and Emotional Well-being

We are utilizing a Rapid Re-Housing model to provide immediate housing supports as well as services to faciliate long-term housing stability for youth who have aged out of care. PATHS is a companion program to our Transitional Resource Agency for Independent Living (TRAIL) and serves the same population. The two program will work very closely together to provide empowering supports to program participants.
